What Is Age Friendly Care and Why Does It Matter?

When families begin exploring home care, they often ask the same question:

"What makes one home care company different from another?"

It's a fair question.

After all, when you're inviting someone into your home to care for a parent, spouse, or loved one, you want to know they're receiving more than basic assistance. You want to know they're receiving thoughtful, personalized care from a team that truly understands the unique needs of older adults.

That's one of the reasons we pursued CHAP Age Friendly Care at Home certification.

For us, it wasn't about adding another badge to our website.

It was about making sure the care we provide is centered around what matters most to the people we serve.

What Is Age Friendly Care?

Age Friendly Care is an evidence-based approach designed specifically for older adults.

Rather than focusing only on tasks or medical conditions, Age Friendly Care looks at the whole person and the factors that have the greatest impact on their quality of life, independence, and overall well-being.

This approach is built around four key areas known as the 4Ms:

What Matters

Every person has different goals, preferences, routines, and priorities.

Age Friendly Care starts by understanding what matters most to the individual receiving care.

For one person, that may mean continuing to attend church every Sunday.

For another, it may mean remaining active in their community, spending time with family, or simply staying in the home they've loved for years.

Medication

As we age, medications can affect balance, energy levels, memory, and overall health.

Age Friendly Care encourages ongoing awareness of medication-related concerns and helps ensure care decisions support a person's overall well-being.

Mentation

Mentation focuses on cognitive and emotional health.

This includes memory, mood, depression, anxiety, dementia, and other factors that can affect quality of life.

Because healthy aging involves more than physical health, emotional and cognitive wellness are an important part of the conversation.

Mobility

Mobility plays a major role in maintaining independence.

Staying active and moving safely can help reduce fall risks while allowing seniors to continue participating in the activities they enjoy.

Where Did Age Friendly Care Come From?

Age Friendly Care is part of a national movement to improve care for older adults.

The Age-Friendly Health Systems initiative was developed by The John A. Hartford Foundation and the Institute for Healthcare Improvement, with support from leading healthcare organizations across the country.

Their goal was simple:

Help older adults receive care that focuses on safety, dignity, independence, and quality of life.

The 4Ms Framework has since become a nationally recognized approach used to improve outcomes and create a better care experience for seniors.

Why We Pursued CHAP Age Friendly Care at Home Certification

One thing many families are surprised to learn is that non-medical home care agencies in Arizona are not required to be licensed by the state.

Because of that, families often have to look beyond minimum requirements and evaluate an agency's training, standards, certifications, and commitment to quality.

At Senior Helpers, we believe seniors deserve more than the minimum.

That's why we chose to pursue CHAP Age Friendly Care at Home certification.

This certification demonstrates our commitment to evidence-based practices that focus on the whole person, not just their immediate care needs.

For us, it reflects our dedication to providing care that is thoughtful, personalized, and designed to support independence whenever possible.

What Does This Mean for Families?

For families in Gilbert, Chandler, Tempe, Maricopa, Ahwatukee, and throughout the East Valley, Age Friendly Care provides an added layer of confidence.

It means working with a care team that takes the time to understand your loved one's goals, routines, and preferences.

It means looking at factors that influence long-term quality of life, not just today's challenges.

And it means receiving care from professionals who understand the unique needs of aging adults.

Whether someone needs companionship, personal care, respite care, Parkinson's support, dementia care, or assistance after a hospitalization, Age Friendly Care principles help guide every aspect of the care experience.
