Recovery Care & Readmission Management

Going Home Safe is our proprietary program and comprehensive set of services that provides families and seniors coordinated support to return home safely after a hospital or skilled nursing facility stay.

We provide families a Going Home Safe Plan Work Book and care coordination plan prior to discharge, and a family engagement workshop to prepare for a safe return home.

Transition Services

With any client, we can also provide transition services during and after a transition from home to an Assisted Living or Independent Living facility. We support in-facility transition to help a loved one become acclimated.

We work alongside local hospice agencies to provide a rich combination of home care and innovative hospice therapies. Our Caregivers receive an end-of-life training program and certification

Medication Management: An added support for clients
Our Registered Nurse oversees medication management, and can administer medication for clients on a monthly or daily basis. We work with clients to create a medication log and pill box set up.
