CHOICES is a Tennessee State Medicaid program used to help those in need.

The program focusses on providing long term care for those who do not have a family caregiver or cannot afford to hire someone to provide the care they need. Enrollment in this program is based on the following requirements:

  • Resident of Tennessee
  • Financially qualify for Medicaid long-term care
  • Need to have level of care provided in a nursing home
  • Be age 65 or older or be an adult age 21 or older with a physical disability
  • Be able to receive services safely at home at a cost less than that of institutional care

The services provided by CHOICES vary depending on the need of the member. Some of these services include but are not limited to: homemaker services, personal care visits, in-home respite care, adult day care, assistive technology, home delivered meals, personal emergency response system, pest control and community based residential alternatives.
