The first two weeks after discharge are the most critical for a successful recovery. Senior Helpers Transitional Care Programs are built precisely for this period.

Transitional Care Programs – Pick the option that works best for you*:

Safety Program: $3800 (a $4650 value)

  • Days 1 – 2 Round the clock care
  • Days 3 – 7 Twelve hours of care each da
  • Days 8 – 11 Six hours of care each da
  • Days 12 – 14 Three hours of care each day

Continuity Program: $1800 (a $2200 value)

  • Days 1 – 14 Five hours of care each day

Sunrise and Sunset Assistance Program: $1200 (a $1500 value)

  • Days 1 – 14 One and a half hours of care each morning and evening.

Each Program also incudes:
